Donaldson Run Dive Team

Please ONLY check the box below if your family is a Late Summer Member of DRRA and has not paid the $100 pool usage fee to the swim team for this season.

As Parent/Guardian of the above-name minor(s), I grant permission for the swimmer(s) to participate in all activities of the DRRA Dive Team, a Northern Virginia Swimming League (NVSL) member team. I represent and warrant that my minor child/children participating on the DRRA Dive Team are in good health and have no physical condition, ailment or disability which renders them unable to participate in vigorous physical activity.For and in consideration of benefits derived from participation in the DRRA Dive Team program, I understand that the risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.I assume all risks and hazards arising out of, or related to, such participation, including, but not limited to, transportation to and from such activities, and do hereby indemnify, release and hold harmless the DRRA Dive Team, its coaches, team representatives, volunteers, property manager, employees and agents, as well as the Northern Virginia Swimming League, from all claims of any kind whatsoever which may arise or hereafter accrue in connection with my child’s/children’s participation in activities of the DRRA Dive Team. I further grant permission for first aid to be given to my child/children in an emergency and will be solely responsible for any medical costs which may arise. I AGREE THAT THEY WILL ABIDE BY THE NVSL Standards of Conduct.

The DR Dive Team may use photos taken throughout the season on its website, in its newsletter and social media platforms. While most of these are private communications per settings imposed on our website, there may be photos circulated more widely on our social media platforms. Please let us know if you allow permission to post your child's photo to these sites by acknowledging the below release:

I, the undersigned, do hereby grant permission to the Donaldson Run Dive Team to post my and /or my child’s photo to the Donaldson Run Dive Team website and/or social media platforms (ie Facebook and Instagram accounts).

I hereby release the Donaldson Run Dive Team, the Donaldson Run Recreation Association and any managers, coaches, members, officers, parents, volunteers from all claims and demands arising out of or in connection with use of photos for dive team related purposes, including without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation, and any other personal and/or property rights.
